Health Coach / Patient Care Coordinator
Location: Hybrid / In-office (preferred)
Schedule: Part-time, 10–20 hours per week
Compensation: $30–45/hour (based on experience) + performance bonuses
Reports to: Podiatric Physician (Practice Owner)
About the Practice
We are a direct-care podiatry practice specializing in non-surgical, regenerative, and integrative solutions for patients with chronic foot and ankle pain who have failed prior treatments and want to avoid surgery.
Our patients are thoughtful, motivated, and seeking high-touch, relationship-based care—not rushed, insurance-driven visits.
Role Summary
The Health Coach / Patient Care Coordinator serves as the primary guide and educator for patients enrolled in our program.
This role focuses on:
patient education
care coordination
nutrition and lifestyle support
accountability and follow-through
This position does NOT diagnose, prescribe, interpret labs, or make medical decisions.
Key Responsibilities: Patient Education & Support
Reinforce the physician’s care plan in clear, patient-friendly language
Review lab results after physician interpretation and explain lifestyle implications
Provide nutrition, stress, sleep, and movement guidance within defined protocols
Support patients through behavior change and adherence
Care Coordination
Track labs, supplements, laser sessions, and follow-up appointments
Ensure patients complete required steps on time
Serve as the main point of contact for non-clinical questions
Communicate patient updates and concerns to the physician
Program Continuity & Experience
Maintain consistent check-ins with patients
Monitor milestones, progress, and barriers
Reduce drop-off and confusion between visits
Uphold a calm, confident, premium patient experience
Administrative Support
Maintain patient notes and tracking tools
Coordinate scheduling and follow-ups related to the program
Assist with onboarding new program patients
What This Role Does NOT Do
Diagnose or treat medical conditions
Order or interpret labs independently
Recommend medications, injections, or medical procedures
Replace the physician’s clinical role
Qualifications
Required
Certification or formal training as a Health Coach, Nutrition Coach, or similar
Strong communication and emotional intelligence
Comfort working with patients who have chronic pain or complex histories
Alignment with holistic, integrative, and regenerative care principles
Confidence working in a cash-based, premium care environment
Preferred
Experience in functional or integrative medicine settings
Familiarity with common lab panels (metabolic, inflammatory, nutritional)
Background in patient navigation or care coordination
Experience supporting lifestyle and behavior change
